- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is my co-pay, if any?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?

Cancellation Policy: If you do not show for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, a cancellation fee may be charged. We manage our appointment times for the benefit of you the Client. Not showing without this notification may prevent us from serving our other clients in a timely fashion. We appreciate our Clients cooperation in this regard.
